Request for Disclosure of Credit History and Inquiries
Gentlemen:
Request is hereby made for your disclosure to me of the nature and substance of
all information, other than medical, presently in your files, pertaining to my credit history.

I also request disclosure of all sources of said information, and the names of the firms
which have requested a consumer report on me and have been furnished same by you
